It is important that the practice has your up-to-date address and phone number so we can contact you in case of an emergency or for routine matters.
If your address or phone number has changed, please inform the practice in writing as soon as possible. You can do this by completing a simple form at the surgery.
If you are notifying us of a name change, please visit the surgery with the necessary legal documentation to support this.
